Relational Quantum Mechanics is an interpretation of quantum mechanics proposed by Carlo Rovelli. Rovelli argues that, in the same spirit as Einstein’s theory of relativity, physical quantities can only have definite values relative to an observer. Relational Quantum Mechanics is hereby able to offer a principled explanation of the problem of nested measurement, also known as Wigner’s friend. Since quantum states are taken to be relative states that depend on both the system and the observer, there is no inconsistency in (...) the descriptions of the observers. Federico Laudisa has recently argued, however, that Rovelli’s description of Wigner’s friend is ambiguous, because it does not take into account the correlation between the observer and the quantum system. He argues that if this correlation is taken into account, the problem with Wigner’s friend disappears and, therefore, a relativization of quantum states is not necessary. I will show that Laudisa’s criticism is not justified. To the extent that the correlation can be accurately reflected, the problem of Wigner’s friend remains. An interpretation of quantum mechanics that provides a solution to it, like Relational Quantum Mechanics, is therefore a welcome one. (shrink)

Previous research has shown that women are disproportionately negatively affected by a variety of socio-economic hardships, many of which COVID-19 is making worse. In particular, because of gender roles, and because women’s jobs tend to be given lower priority than men’s (since they are more likely to be part-time, lower-income, and less secure), women assume the obligations of increased caregiving needs at a much higher rate. This unfairly renders women especially susceptible to short- and long-term economic insecurity and decreases in (...) wellbeing. Single-parent households, the majority of which are headed by single mothers, face even greater risks. These vulnerabilities are further compounded along the dimensions of race, ethnicity, class, and geography. Drawing upon the philosophical literature on political responsibility and structural injustice (specifically, the work of Iris Marion Young), I argue that while the state may not have had either foresight into, or control over, the disproportionate effect the pandemic would have on women, it can nonetheless be held responsible for mitigating these effects. In order to do so, it must first recognize the ways in which women have been affected by the outbreak. Specifically, policies must take into account the unpaid labor of care that falls on women. Moreover, given that this labor is particularly vital during a global health pandemic, the state ought to immediately prioritize the value of this work by providing financial stimuli directly to families, requiring employers to provide both sick leave and parental leave for at least as long as schools and daycares are inoperational, and providing subsidized emergency childcare. By the time I graduated high school I’d hit 250 lbs.Even (...) today, after losing a considerable amount of weight from having Roux–en–Y gastric bypass surgery, I am still considered overweight according to the body mass index.All this is to say that the way I think about my health, my body and my life is very much from the perspective of someone who has never experienced the so–called condition of being “normal.”Enough is EnoughI’ve been asked many times how I decided to have bariatric surgery. The answer isn’t simple. Because I’d been overweight all my life, the notion of being anything but overweight seemed a bit absurd to me. Not absurd as in, “I would not be happy if I were smaller” but more like, “I don’t know if this could ever happen for me and, if it did, who would I be?”Truth be told, I wasn’t entirely unhappy as an overweight woman. By the account of my primary care physician I also was not necessarily unhealthy for an overweight person. I didn’t have high blood pressure or diabetes (I wasn’t even pre–diabetic). My heart seemed to work fine. I was just very, very large.However, I’d been large (or larger than others) my entire life. In many ways my very identity was inextractable from that fact. Being too thin was always presented to me as a negative cultural value, I believe because it was associated with drug use.That means that growing up I didn’t have some of the experiences other obese people had. There were a few people who teased me but they were the exception, not the norm. I don’t recall being lonely because of my size. I dated avidly throughout my adolescence. I was asked to, and went to, both my proms.I proceeded into young adulthood with a few bumps along the way. I suffered a bout of depression in my sophomore year of college and just after my 21st birthday I found the first signs of what would be massive hair loss from alopecia areata (although did not know that’s what it was at the [End Page 107] time). This paper examines the ways in which two methods of horse training generally considered divergent approach the concepts of partnership and conflict in human-horse relations. It focuses on finding similarities between the methods, both of which, it is argued, demonstrate the significance of communication in improving human-horse relations. Using interview material, the paper analyzes the practices and beliefs of individuals involved in natural horsemanship. In doing so the paper shows that communication between human and horse works to promote relations between (...) them that are free of conflict. This analysis is offered as a potential “solution” to welfare problems that exist both within competitive dressage practice and within individual human-horse relations. The paper also examines how the fundamental similarities between dressage and natural horsemanship could point to universalized theories of horse training focused on improving human-horse relations. Despite recent increases in research on emotions and regulation in collaborative learning, measuring both constructs remains challenging and often lacks structure. Researchers need a systematic method to measure both the formation of emotions and subsequent regulation in collaborative learning environments. Drawing from the Formation and Regulation of Emotions in Collaborative Learning model, I introduce a new observational coding procedure that provides comprehensive guidelines for coding these phenomena. The FRECL coding procedure has been implemented successfully in other studies and is described (...) here in detail. Specifically, I detail the ideal situations for using the procedure, discuss background information and present a codebook and empirical examples for each stage of the FRECL model, and provide additional considerations that allow researchers flexibility based on their own experiences and preferences. This procedure extends past research by providing an accessible observational protocol that is both systematic and comprehensive. The FRECL coding procedure can benefit future research by providing more organized consistency to the measurement of collaborative emotions and regulation. (shrink)

Brain-computer interfaces are communication bridges between a human brain and external world, enabling humans to interact with their environment without muscle intervention. Their functionality, therefore, depends on both the BCI system and the cognitive capacities of the user. Motor-imagery BCIs rely on the users’ mental imagination of body movements. However, not all users have the ability to sufficiently modulate their brain activity for control of a MI-BCI; a problem known as BCI illiteracy or inefficiency. The underlying mechanism of this phenomenon (...) and the cause of such difference among users is yet not fully understood. In this study, we investigated the impact of several cognitive and psychological measures on MI-BCI performance. Fifty-five novice BCI-users participated in a left- versus right-hand motor imagery task. In addition to their BCI classification error rate and demographics, psychological measures including personality factors, affinity for technology, and motivation during the experiment, as well as cognitive measures including visuospatial memory and spatial ability and Vividness of Visual Imagery were collected. Factors that were found to have a significant impact on MI-BCI performance were Vividness of Visual Imagery, and the personality factors of orderliness and autonomy. These findings shed light on individual traits that lead to difficulty in BCI operation and hence can help with early prediction of inefficiency among users to optimize training for them. (shrink)

BackgroundDespite its ubiquity, it is often not clear what organizations and services mean by well-being. Visual impairment has been associated with poorer well-being and well-being has become a key outcome for support and services for adults living with VI. A shared understanding of what well-being means is therefore essential to enable assessment of well-being and cross-service provision of well-being support.ObjectivesTo provide an overview of the ways in which well-being has been conceptualized in research relating to adults living with VI.Eligibility criteriaArticles (...) were included in the review if the article discussed well-being in the context of adults living with VI, was available in English and as a full text.Data sourcesA systematic search using search terms relating to VI and well-being was conducted in EBSCOHost and Ovid.ChartingA team of three reviewers screened titles, abstracts and full-texts articles and extracted data. Ambiguous articles were referred to the research group and discussed.ResultsOf 10,662 articles identified in the search, 249 were included in the review. These referred to 38 types of well-being. The most common types were general well-being emotional well-being and psychological well-being. Most articles referred to one type only, with a maximum of 9 listed in one article. A large number of articles did not clearly define well-being. A wide range of indicators of well-being related to the domains of hedonia, mood, positive and negative affect, quality of life, mental health, eudaimonia, self/identity, health, psychological reactions to disability and health problems, functioning, social functioning and environment, were extracted, many of which were used just once.ConclusionsThere remains a lack of consensus on how well-being is conceptualized and assessed in the context of adult VI. A standardized multi-domain approach derived with input from adults with VI and practitioners working with them is required to enable comparison of findings and cross-organizational provision of support. There has been considerable debate on how to teach ethics within the marketing curriculum to accommodate the AACSB requirements requiring emphasis on ethical issues within the business curricula. Since introducing a separate course on marketing ethics has limited reach, we propose incorporating the ethical dimension through guest speeches and reflective learning in a mandatory Marketing course for all business majors. Through phenomenographic analysis of 121 student reflections, we report evidence supporting the effectiveness of this approach in significantly raising awareness of (...) ethics in the marketing domain and in initiating the nurturing of ethical reasoning abilities in undergraduate business students early in their college careers. Students absorbed substantial amounts of knowledge at different levels and demonstrated significant cognitive processing within the revised Bloom’s taxonomy. Using this method to teach marketing ethics promotes awareness and application of ethical reasoning without compromising disciplinespecific instruction. (shrink)

Most consumers are aware that climate change is a growing problem and admit that action is needed. However, research shows that consumers’ behavior often does not conform to their value and orientations. This value-behavior gap is due to contextual factors such as price, product design, and social norms as well as individual factors such as personal and hedonic values, environmental beliefs, and the workload capacity an individual can handle. Because of this conflict of interest, consumers have a hard time identifying (...) the true drivers of their behavior, as they are either unaware of or unwilling to acknowledge the processes at play. Therefore, consumer neuroscience methods might provide a valuable tool to uncover the implicit measurements of pro-environmental behavior (PEB). Several studies have already defined neurophysiological differences between green and non-green individuals; however, a behavior change intervention must be developed to motivate PEB among consumers. Motivating behavior with reward or punishment will most likely get users engaged in climate change action via brain structures related to the reward system, such as the amygdala, nucleus accumbens, and (pre)frontal cortex, where the reward information and subsequent affective responses are encoded. The intensity of the reward experience can be increased when the consumer is consciously considering the action to achieve it. This makes goal-directed behavior the potential aim of behavior change interventions. This article provides an extensive review of the neuroscientific evidence for consumer attitude, behavior, and decision-making processes in the light of sustainability incentives for behavior change interventions. Based on this review, we aim to unite the current theories and provide future research directions to exploit the power of affective conditioning and neuroscience methods for promoting PEB engagement. The text of these historical documents is translated with great attention to style and accuracy, and generous editorial scaffolding gives glimpses into the writers’ world. Nikki Lane Bringing Flesh to Theory: Ethnography, Black Queer Theory, and Studying Black Sexualities As Dorothy Hodgson tells us, the most common features of an ethnographic project involve “talking to, participating with, and observing the people who produce... texts, exploring the contexts of their ideas and actions, and often studying how their situations, ideas, and actions (...) change over time.”1 Practically, this often involves spending months, and more often years, in a particular field site, where one develops relationships with members of the group, community, or institution being studied. Ethnographers are positioned in a place to observe, but also place their bodies on the line—participating, when possible, in the quotidian practices of the group. This observation and participation is captured in the form of “field notes” that may relay in the form of “thick description,” what the ethnographer sees as she observes and participates in various cultural and social practices.2 As such, ethnography requires reflexivity because the ethnographer must constantly consider how her body is affecting and is effected by the communities and institutions in which she is embedded. The benefit of this reflexive ethnographic approach is that, as Faye Ginsburg notes, it “has the capacity to reveal the fault lines in 1. What links these projects is their use of ethnographic methodologies to understand how Blackness informs racialized gender and sexuality in the everyday experiences of their interlocutors. In relying on ethnography, to varying degrees, they are in conversation with and expand upon methodological trends within Black feminist studies and Black queer studies. Further, the Black sexuality studies projects reviewed here question the (hetero)normative bent within the field of African American studies, the normatively white subject position that exists within queer theory, and the lack of attention to issues of sexual pleasure within Black feminist theory. In adolescence, aspects of cognition that are required to deal with complex cooperation situations, such as mentalising and social value orientation, are still in development. In the Trust Game, cooperation may lead to better outcomes for both players, but can also lead to exploitation by the trustee. In the present study, we explore how mindreading, a crucial aspect of mentalising, and social value orientation (whether someone is prosocial or proself) are related to trust. In a group of 217 students (51% (...) girls, Mage = 15.1) social value orientation, mindreading and trust (using the Trust Game) were measured. The result show that social value orientation moderates the relation between mindreading and trust. In the group of prosocials, we find no correlation between mindreading and trust. In the group of proselfs, mindreading is negatively correlated to trust, indicating that proselfs use their mentalising skills to assess that the trustee is likely to exploit them. (shrink)

Zonulin is a physiological modulator of intercellular tight junctions, which upregulation is involved in several diseases like celiac disease (CeD). The polyQ gliadin fragment binds to the CXCR3 chemokine receptor that activates zonulin upregulation, leading to increased intestinal permeability in humans. Here, we report a general hypothesis based on the structural connection between the polyQ sequence of the immunogenic CeD protein, gliadin, and enteric coccidian parasites proteins. Firstly, a novel interaction pathway between the parasites and the host is described based (...) on the structural similarities between polyQ gliadin fragments and the parasite proteins. Secondly, a potential connection between coccidial infections as a novel environmental trigger of CeD is hypothesized. Therefore, this report represents a promising breakthrough for coccidian research and points out the potential role of coccidian parasites as a novel trigger of CeD that might define a preventive strategy for gluten‐related disorders in general. By comparing two time periods, the early and late 20th century, this article examines the ambiguities and ambivalences in the state promotion of women in the nation-building projects of Mexico. I argue that in both cases, the state was keen to promote itself as modern and progressive and used women's status in society to these ends. Despite the explicit focus on women, there were many ambiguities and ambivalences resulting from the competing state projects in the political, socio-economic and cultural arenas (...) offering women both privileged spaces and constraints in the development of gendered citizenship. The contradictions arise from simultaneously promoting women's rights, extolling traditional gender roles and fearing women's political activism – both conservative and more radical. Although these ambivalences and ambiguities remain a constant feature, there is a key difference in the two time periods: in one the regime is inward looking, economically protectionist and corporatist, while in the other a new vision of Mexico has attempted to dismantle the corporatist structures and state development project with private economic initiatives and political individualism. In both periods, women gained important rights but romanticized imagery of the self-sacrificing mother was mobilized to underpin change: women were expected both to change and remain the same. In 1977 John Money published the first modern case histories of what he called ‘apotemnophilia’, literally meaning ‘amputation love’ [Money et al., The Journal of Sex Research, 13(2):115–12523, 1977], thus from its inception as a clinically authorized phenomenon, the desire for the amputation of a healthy limb or limbs was constituted as a sexual perversion conceptually related to other so-called paraphilias. This paper engages with sex-based accounts of amputation-related desires and practices, not in order to substantiate the paraphilic model, but (...) rather, because the conception of these (no doubt) heterogeneous desires and practices as symptoms of a paraphilic condition (or conditions) highlights some interesting cultural assumptions about ‘disability’ and ‘normalcy’, their seemingly inherent (un)desirability, and their relation to sexuality. In critically interrogating the socio-political conditions that structure particular desires and practices such that they are lived as improper, distressing and/or disabling, the paper constitutes an exercise in what Margrit Shildrick [Beyond the body of bioethics: Challenging the conventions. AimTo examine the relative cost-effectiveness of predictive genetic tests for familial breast and ovarian cancer provided by Genetic Services of Western Australia.MethodsThe relative cost-effectiveness was assessed using a decision analytic model.ResultsThe cost and outcomes of genetic testing was compared in first-degree relatives of known BRCA1/2 mutation-carriers who have a 50% risk of carrying the mutated gene (intervention group) to individuals with the same a priori risk but who do not undergo a genetic test (control subjects).Since genetic testing enables the restriction (...) of intensive surveillance to individuals with an identified BRCA1/2 gene mutation, net savings in the period observed (age 25-70) were $980-$1008 per woman in the ovarian intervention group and $1681-$1795 per woman in the breast intervention group, and delayed the onset of breast cancer (6mths BRCA1, 3mths BRCA2).Compared to control subjects undergoing population surveillance, it was estimated the onset of breast cancer could be delayed at a total net cost of $3055 (5.1yrs) to $3389 (3.2yrs) for women in the breast intervention group with BRCA1/2 mutations. Since population surveillance is not currently recommended for ovarian cancer, control subjects undergoing no surveillance were compared with the intervention group. The onset of ovarian cancer was delayed at a net cost of $1630 (3.5yrs) to $2509 (1.2years) for women with BRCA1/2 mutations.ConclusionsTesting allows targeted high-level surveillance for gene mutation carriers, which ensures the cost-effective use of resources and reduces cancer-related morbidity if clinical recommendations for intervention are adopted. Reproduction and immunity are energy intensive, intimately linked processes in most organisms. In women, pregnancy is associated with widespread immunological adaptations that alter immunity to many diseases, whereas, immune dysfunction has emerged as a major cause for infertility in both men and women. Deciphering the molecular bases of this dynamic association is inherently challenging in mammals. This relationship has been traditionally studied in fast‐living, invertebrate species, often in the context of resource allocation between life history traits. More recently, these studies (...) have advanced our understanding of the mechanistic underpinnings of the immunity‐fertility dialogue. Here, we review the molecular connections between reproduction and immunity from the perspective of human pregnancy to mechanistic discoveries in laboratory organisms. We focus particularly on recent invertebrate studies identifying conserved signaling pathways and transcription factors that regulate resource allocation and shape the balance between reproductive status and immune health. Recently, evidentialists have argued that only they can explain transparency--the psychological phenomena wherein the question of doxastic deliberation of whether to believe p immediately gives way to the question of whether p--and thus that pragmatism about epistemic reasons is false. In this paper, we provide a defense of pragmatism. We depart from previous defenses of pragmatism which argue against the evidentialist explanation of transparency or the fact of transparency itself, by instead arguing that the pragmatist can provide a sound explanation (...) of transparency while maintaining her distinctive view of epistemic reasons. We do so by putting forth an evolutionary debunking explanation of doxastic deliberation. Since both the evidentialist and pragmatist have sound explanatory accounts of transparency, we argue that transparency should not be appealed to when trying to adjudicate the debate between evidentialists and pragmatists, but rather must be decided on other grounds. On January 29, 2021, a police officer with the Rochester, New York, Police Department pepper-sprayed a 9-year old Black girl who had been handcuffed and forced into the back of a police car. In the struggle that proceeded this moment, an officer yelled at the girl with obvious frustration, “You’re acting like a child!” In this essay, I consider how the girl’s quick retort —“I AM a child!”—interjected a truth into the struggle that had been all but ignored by the (...) armed adults on the scene. I consider how the truth embedded in this girl’s call exposes the lies of law enforcement and, in doing so, lay the seeds of abolitionist imaginings—a call for a system, a world, that would treat a Black girl as if she were a child. Advances in genomic medicine have lead to debate about the potential inclusion of genetic tests for susceptibility to common complex disorders in newborn screening programmes. Empirical evidence concerning psychosocial reactions to genetic testing is a crucial component of both ethical debate and policy development, but while there has been much speculation concerning the possible psychosocial impact of screening newborns for genetic susceptibilities, there remains a paucity of data. The aim of the study reported here is to provide some of this (...) missing empirical evidence, using type 1 diabetes as an example of a common disorder with multiple significant genetic contributors to its aetiology. Semi-structured interviews were conducted with 11 parents of babies who had received increased risk results in a study that involved newborn screening for genetic susceptibility to type 1 diabetes. Interpretative phenomenological analysis was used to evaluate the data. The interview data suggest that the probabilistic nature of results of genetic susceptibility tests impacts upon all aspects of parents' psychosocial reactions, resulting in a complex and dynamic process quite different to that described in relation to current newborn screening programmes. While parents generally reported fairly minor levels of concern in response to news of their child's increased genetic risk, these worries frequently recurred, and perception of risk also varied and fluctuated over time. Both individual and contextual factors appeared to interact with the inherent uncertainty of the test result to contribute to the dynamic nature of parental reactions, and their behavioural responses. The implications of these findings for future research and for the debate concerning potential expansion of newborn screening are discussed. (shrink)

Treating physicians have key roles to play in expanded access to investigational drugs, by identifying investigational treatment options, assessing the balance of risks and potential benefits, informing their patients, and applying to the regulatory authorities. This study is the first to explore physicians’ experiences and moral views, with the aim of understanding the conditions under which doctors decide to pursue expanded access for their patients and the obstacles and facilitators they encounter in the Netherlands. In this mixed-methods study, semi-structured interviews (...) and a questionnaire were conducted with medical specialists across the country and analysed thematically. Typically, our respondents pursue expanded access in “back against the wall” situations and broadly support its classic requirements. They indicate practical hurdles related to reimbursement, the amount of time and effort required for the application, and unfamiliarity with the regulatory process. Some physicians are morally opposed to expanded access, with an appeal to safety risks, lack of evidence, and “false hope.” Some of these moral concerns and practical obstacles may be essential targets for change, if expanded access to unapproved drugs is to become available for wider groups of patients for whom standard treatment options are not—or no longer—available, on a more consistent and equal basis. Drawing from a long history of feminist writing grounded in personal reflection and informal dialogue between feminist thinkers, Cobb and Godden-Rasul present an email-based conversation with Jess Lishak, the outgoing Women’s Officer at the University of Manchester Students’ Union. The conversation draws on Cobb and Godden-Rasul’s experience as feminist academics engaged in critical institutional practice through such initiatives as editing the Inherently Human blog, organising the Inspirational Women of Law exhibition, and participating in university working groups on campus-based harassment and (...) violence. In asking Lishak to reflect on her journey to feminism and her experiences of activism, the conversation ranges over such issues as personal influences and experiences, strategies for securing institutional support, encouraging student engagement with feminism, and campaigning tactics. The conversation developed out of a “Campus Feminisms” event in March 2016, which explored the rise of exciting new grassroots single-issue campaigns and political mobilisations by students in higher education, and was organised by Cobb and Godden-Rasul at Newcastle University, UK. Undergraduate and postgraduate students shared their personal struggles and achievements in bringing feminist ideas and campaigns to their university campuses. Lucy Morgan, the Gender Equality Officer at Newcastle University Students’ Union, offered inspiring reflections on her efforts to reinvigorate the ‘F’ word, in the face of simultaneous student apathy and backlash. Many of these campus-based mobilisations have demanded better institutional responses to sexual violence against women. At around the same time, Cobb was beginning a new role as the co-chair of the University of Manchester’s first Task & Finish Group on Sexual Violence and Harassment on Campus. This followed Universities UK’s decision to create a taskforce to consider options for improving institutional responses to student safety. In the process, Cobb crossed paths with Lishak, who had been appointed a member of the UUK Taskforce in light of her path-breaking students’ union work addressing violence against women. Since Lishak was an exemplar of this new feminist wave in higher education, one that was still inadequately understood by feminist academics despite often working side-by-side within the same institutions, the authors embarked on this conversation in order to better understand the relationship between academic and student feminist activism on campus. As Lishak makes clear in her own reflections, there is nothing inevitable about the synergies between these movements, but there is potentially a great deal that could be achieved through their closer engagement. It seeks to understand his contribution against the developments within the preceding Greek and Arabic intellectual milieus, and to appreciate his philosophy as such by emphasising his independence as a critical and systematic thinker. Exploring Avicenna’s method of "teaching and learning," it investigates the implications of his account of the natural body as a three-dimensionally extended composite of matter and form, and examines (...) his views on nature as a principle of motion and his analysis of its relation to soul. Moreover, it demonstrates how Avicenna defends the Aristotelian conception of place against the strident criticism of his predecessors, among other things, by disproving the existence of void and space. Finally, it sheds new light on Avicenna’s account of the essence and the existence of time. For the first time taking into account the entire range of Avicenna’s major writings, this study fills a gap in our understanding both of the history of natural philosophy in general and of the philosophy of Avicenna in particular. Social science requires a dual ontology: one for the physical realm, and one for the symbolic realm of meaning. Much research produced in social science remains based in an old paradigm, which entirely neglects the symbolic realm. While social scientists attempting to forge a new paradigm have embraced a discursive approach, this approach lacks a coherent framework that can be systematically applied in the analysis of meaning. This paper presents the positioning diamond as a framework that can be employed in (...) discourse analysis across social science disciplines. The four facets of the diamond—storylines, identities, rights and duties, and the social force of acts—can be analyzed at three levels of discourse: the content, narrator-interlocutor, and ideological levels. This book acknowledges and highlights the moral excellence embedded in black queer practices of family. Taking the lives, narratives, and creative explorations of black queer people seriously, Thelathia Nikki Young brings readers on a journey of new, queer ethical methods that include confrontation, resistance, and imagination. Young asserts that family and its surrounding norms are both microcosms of and foundations for human relationships. She discusses how black queer people are moral subjects whose ethical reflection, lived experience, and embodied action (...) demonstrate valuable moral agency for those of us thinking about liberating and life-giving ways to enact “family.” Young posits that black queer people enact moral agency in ways that ought to be understood qua moral agency. Refusing to recognize the examples from this (and any other) community, Young argues, denies us all the learning and moral growth that come from connecting with diverse human experiences. This book investigates how acknowledging and critically engaging with the moral agency within marginalized subjectivities allow us to consider and bear witness to the moral potential in us all. Background Patients with unmet medical needs sometimes resort to non-standard treatment options, including the use of unapproved, investigational drugs in the context of clinical trials, compassionate use or named-patient programs. The views and experiences of patients with unmet medical needs regarding unapproved, investigational drugs have not yet been examined empirically. Methods In this qualitative study, exploratory interviews and focus groups were held with patients with chronic or life-threatening diseases, about topics related to non-standard treatment options, such as the search for (...) non-standard treatment options, patients’ views of the moral obligations of doctors, and the conditions under which they would or would not wish to use non-standard treatment options, including expanded access to unapproved, investigational drugs. Results Respondents had very little knowledge about and/or experience with existing opportunities for expanded access to investigational drugs, although some respondents were actively looking for non-standard treatment options. They had high expectations of their treating physicians, assuming them to be aware of non-standard treatment options, including clinical trials elsewhere and expanded access programs, and assuming that they would inform their patients about such options. Respondents carefully weighed the risks and potential benefits of pursuing expanded access, citing concerns related to the scientific evidence of the safety and efficacy of the drug, side effects, drug-drug interactions, and the maintaining of good quality of life. Respondents stressed the importance of education and assertiveness to obtain access to good-quality health care, and were willing to pay out of pocket for investigational drugs. Patients expressed concerns about equal access to new and/or non-standard treatment options. Conclusion When the end of a standard treatment trajectory comes into view, patients may prefer that treating physicians discuss non-standard treatment options with them, including opportunities for expanded access to unapproved, investigational drugs. Although our respondents had varying levels of understanding of expanded access programs, they seemed capable of making well-considered choices with regard to non-standard treatment options and had realistic expectations with regard to the safety and efficacy of such options. Dutch patients might be less likely to fall prey to false hope than often presumed. (shrink)

It is rather natural to suppose that what we mean when we say that an institutional organization has a moral duty is parallel to whatever it is that we mean when we say that an individual has a duty. I challenge this interpretation on the grounds that it assumes that institutional organizations possess those characteristics or abilities requisite for moral agency—an assumption which I argue is highly suspicious. Against such an interpretation, I argue that we have very good reasons to (...) suppose that the term 'has a duty' is used equivocally across individual and institutional contexts. In other words, the meaning of an institutional duty-claim is quite different than that of an individual duty-claim, so much so that we ought to recognize that institutional duty-claims are not really duty-claims at all. Building on an action theoretical perspective, it is assumed that most television viewing is a routine response to frequently occurring situations, which together make up everyday life. This interplay between television viewing and everyday life was studied using data from a national survey among Dutch adults and their families. From this survey, data of 225 couples were analyzed using event history analysis. Results indicate that one cannot see television viewing as merely an alternative for other activities. For instance, participatory activities (...) have two distinct effects: They tend to inhibit television viewing by the actor but stimulate television viewing by the actor’s partner. The effect of contacts with other variables appears to be important as well: Being at home, engagement in child care, household work, and eating and drinking often enhances television viewing. But presence of non-family may inhibit television viewing. Education was shown to have a consistently negative impact on television viewing, and there appeared to be some gender specific inducements for termination of television viewing. (shrink)

The blank verse as ›unbound‹ verse has been the object of lively discussions about the freedom of poetry since its emergence in the Renaissance. The basis of these discussions is the status of rhyme. Its absence opens up new possibilities of representation. Against the fetters of rhyme, blank verse is increasingly stylised as a form of freedom in the 18th century, according to the thesis presented here. This article focuses on the discourses on tragic blank verse throughout Europe up to (...) 1800. We were supposed to be in a 'postfeminist' age. But recently we've seen a resurgence of feminist campaigning among women (and some men). There's a new brand of feminism: young, social media savvy, militant. But there's also a new kind of backlash, driven by so-called fundamentalists and by increasingly overt misogyny.
